

Overall, I give the portrayal of schizophrenia in Shutter Island a 0/10 as the character did not have schizophrenia at all.Tobey and his crew manage his late father's garage Marshall Performance Motors. Finally, Teddy’s delusions are not due to substance abuse, but rather exist because of the trauma he experienced when he was in World War II, when his wife drowned their kids, and when he killed his wife. Teddy does indeed function as he normally would, he just functions as he sees himself in his delusion (as a Marshall) not as he is in reality. A third requirement is that the disorder does not stop the person’s level of functioning. Additionally, people who are diagnosed with delusional disorder must have already ruled out schizophrenia, which we have done for Teddy.

Teddy’s lesser delusions that the institution is performing lobotomies and experimenting on patients is also plausible as they were prominent at the time the movie is set. Teddy’s delusions could happen as his wife was killed (however by him) and he was a U.S. These hallucinations must be non bizarre, meaning that they could actually happen. Marshall looking for a missing patient and his wife’s murderer at a mental institution when in reality he is a patient. This is certainly the case with Teddy who believes that he is a U.S. Delusional Disorder is characterized by a person being in a state of psychosis in which they can not tell what is real and what is imagined.

Given this, Teddy’s symptoms more closely point towards Delusional Disorder. While some may argue that he does hallucinate, his beliefs are not just in his head, they are triggered by an external stimulus, which goes against the definition of a hallucination. He does not have negative symptoms, he does not have disorganized speech or motor behavior, and he does not have hallucinations. While Teddy does have delusions, this is the only symptom of schizophrenia that he possesses. The portrayal of schizophrenia in Shutter Island is inaccurate as the main character Teddy does not actually have schizophrenia.
